what's the card rev? hard to read, but it looks like 859-01 REV: A?
I've never heard of production boards with lower card revs than 906-01 REV A.. must be some engineering sample or something?Comment

I's say it's more likely an OEM card for some system integrator who only wanted singlehead functions.

Ad. heatsink - normal for G400 cards I got very similar card (OEM/SH G400 cracked too MAX clocks).
