I'm not familiar with these trucks, where would the operator have been whilst listing the small van?

That is a very good question.
He was using hand leavers on the left side of the crane boom pedistal to control it.
You can just see him behind the left side of the cab.
But, I don't see him in any pictures durring or after the fall.
I sure hope he didn't ride it over!
