    Complete re-install. I'd just remove the drivers and control panel run drive cleaner from driverheaven.net. Then run regcleaner and then re-install the drivers after double checking with regedit.

      That's because you are using the Windows software control, rather than the Radeon hardware control.

      To use the Radeon hardware control and program the refresh rate yourself, click Advanced timing options, and then adjust the "Refresh rate" control under "Vertical geometry" (or the "Pixel clock" control under "RAMDAC"). You'll find it is very precise.
