- Volume. It is ubiquetous as a standard. Practically everone with a PC has to use it for one reason or another, and pretty frequently usually. So any amount of annoyance per user is multiplied by a _lot_ of users.
- Complexity. Almost all of the people forced to use it do not need ninty-five percent of its features. And the features make it slower and harder to do the simple things.
- Bugs. It has the same bugs it did back in the 16bit versions. For example, I still get random changes to fonts that get "stuck" and cannot be removed without deleting large sections of text completely pasting them in notepad then back to Word. And sometimes the formating still comes back! This is most notable with bullet lists. This is just one exampel
- That damn paperclip! No matter how many times you turn him off, he is still lurking, waiting to annoy you when you least expect it.

I'd say any software provided by Creative is a hard contender for this "award".
Real Softwares, creative softwares, Norton anti-viruses also s*ck big times whenever used with some windows 2000 boxes.
Top contenders are Symantec and Real for me, creative being just a pain in the .ss
So many people hate Quicktime and Real, yet so many people use it!!!

Why would you hate PDFs? They're the best way to transfer a document and be sure that both sides see the same layout.

Actualy you are not forced to use quicktime and realplayer you could always use Quicktime and Real alternative available here http://www.freecodecs.com/
