The "I don't use a mouse" option was for tablet or other obscure input device users. I just got a Wacom Graphire3 tablet this afternoon and have been using it instead of a mouse to get used to the pen.
I use a Kensington trackball (inappropriatly named Expertmouse 5).
Accelleration is enabled, with extreme fine adjustments (this allows me to
disable accelleration at low speeds, but have it prominently at high
speeds)
As posted before : navigating multiple displays is difficult otherwise.

Actually... yes...
But I posted from in Lynx, and somehow (I now see) the post appeared twice...
I mean the driver has 2 fine adjustment settings:
1. accelleration when moving slowly
2. accelleration when moving fast
The former is set to minimal, the latter to much much higher... As a result, there is no accelleration when moving slowly, but a lot of accelleration when moving the same trackball fast.

With my M$ Wheel Mouse Optical I can leave the standard XP mouse control panel (no IntelliMouse drivers thank you very much ) at the middle sensitivity and get single pixel accuracy and still cross the entire screen in less than half the width of my bog standard "wetsuit" mouse pad, and without even moving my wrist if so desired (not that that's a good thing to do ergonomically.)
I think the problem I have with MS's mouse acceleration is that it's too jumpy, on my Amiga I had a little mouse acceleration utility that had extremely fluid acceleration that was really pleasant to use.
