Are there any antivirus programs that have a real-time scanner that can be set to only check files accessed by the programs that I specify, like firefox.exe, thunderbird.exe, iexplore.exe, and so on?

NOD32 does. It has various 'scanners' that protect different aspects of the system. The 'Web access protection' portion lists all of your apps and allows you to selectively turn protection on or off, and if it's not on the list you can add it.
