Card Identification

I recently pulled a Mystique out of an old machine. Is there a way to identify which card I have from the markings and tags so I can download the correct drivers and maybe an manual?

Do you have acess to a good scanner? If so, scan it in and post the image here. These fine folks will help you out. Or just jot down all the numbers and names on the chips of the card and someone will recognise it.
