why does she have her arms at her hip, rather than by her sides?
looks as if she's trying to enhance the way her assets look

Marilyn Monroe didn't need to enhance her assets

The Doctor taking the picture though was probably a very happy chappy

She has her shoulders hunched, and probably thrust foreward as well, to move the shoulder blades out of the way to get a clear view of the heart and lungs. Or so I'm told.

How about a nice positive contact print suitable for framing?

You were told semi-correctly: not hunched but just rolled forward to get the shadows of the scapulae out of the lung fields. Placing the hands on the hips helps with immobilization and in moving the elbows to the correct locations.

I also surmise that the x-ray film is on the wall; that she's facing the wall; that the x-ray machine is behind her.

I may just print this one out on a poster printer. That should blow up to 24" x 24" all right, shouldn't it?

Yes, a film holder is attached to a lead-shielded wall and the patient positioned facing it with the x-ray emitter behind them. The correct size for this type of radiograph shot in the US is 14 x 17 inches.
