All nicely done. The last two brought tears.
Both are a tribute to management stupidity, criminal in my opinion.

I have long thought that charges of criminal negligence should have been brought in both the Challenger and Columbia incidents. In both cases there were red flags all over the place, but managers were more focused on schedules and complacency than safety and common sense.
