SpaceX's failures have been quite visible, especially during the early landing tests.

Early on is the explosion of the F9-R Dev-1 landing test vehicle at their McGregor TX test center. A bad engine sensor triggered the self-destruct system.

About 2/3 of the way in is the "Leaning Tower of ThaiCom" which danced all over the drone ship's deck in high seas, leaning because of a crushed shock absorber can in one leg.
