Cards we've actually used for extended periods:
G200 (in my dads computer currently, was also the card that made me swear to never buy another Matrox card after it took them an eternity to replace when it's BIOS malfunctioned in an irreversable way.)
G400 MAX (was bought as an upgrade to the G200, thus breaking the "never buy Matrox again" vow.
now sitting in an anti-static bag on a shelf)
Radeon 8500, 64 meg 275/275 "classic" version (in my current system)

Voodoo 3 2000
GeForce 3 TI-200
GeForce 4 TI-4400
The C=64 and the Amiga's we've owned didn't have "video cards" so I'm not counting them.
