Cards we've actually used for extended periods:

G200 (in my dads computer currently, was also the card that made me swear to never buy another Matrox card after it took them an eternity to replace when it's BIOS malfunctioned in an irreversable way.)

G400 MAX (was bought as an upgrade to the G200, thus breaking the "never buy Matrox again" vow. now sitting in an anti-static bag on a shelf)

Radeon 8500, 64 meg 275/275 "classic" version (in my current system)

    Have had at various times:

    Matrox Mystique
    Voodoo 3 2000
    GeForce 3 TI-200
    GeForce 4 TI-4400

The C=64 and the Amiga's we've owned didn't have "video cards" so I'm not counting them.

              Hmm, let's see:

              Matrox G100 Productiva 8MB
              Creative Riva TNT 16MB
              Creative GeForce256 32MB SDR
              Creative GeForce256 32MB DDR
              Leadtek GeForce256 32MB SDR
ATI Rage Fury PRO VIVO 32MB SDR
              Inno3D GeForce2 MX400 64MB SDR
Nvidia Riva128ZX 8MB (currently rotting in my PC in Romania )
Matrox Millennium I 4MB (only PCI card I've ever used...as recently as last year, mind you, when my Parhelia was being serviced)
              Matrox Parhelia 128MB AGP 4x

At my office I have an ELSA GeForce 4MX440 (64MB DDR) and an onboard ATI Rage XL 4MB in my server (incredibly crappy image quality on that one)

It would be a long list, and I can't remember all of them but I'll cut it short:

various S3's in VLB, PCI, AGP flavors + some Tridents and Tsengs in the olden days...
                            Matrox Millenium I
                            Matrox Mystique 170
                            Matrox Millenium II
                            NoName Voodoo I
                            Matrox G200
                            Matrox G400 16Mb SH
                            Matrox G400 MAX
                            Matrox G450
                            Matrox G550
                            ATI Radeon 9000Pro AIW
                            ATI Radeon 9600Pro AIW
                            ATI Radeon 9800Pro
SiS 650 integrated in my laptop

Of the above G400MAX, G400 16SH, G550, ATI 9600AIW, ATI 9800Pro and SiS650 are still chugging along in various machines I have at home...
