Wow, I got in late in this discussion. I have used driver version 5.20.016 and 5.03. Both gave me dual head capability under W2K.
Now that I've thought about it, have you tried switching monitor cables? However you switched monitors, with their cables or not, have you tried the other orientation? That is switching the monitors and their respective cables as one if you didn't previously.
Are you using BNC cables? If so, try a D-Sub cable for at least the problem monitor.
