According to the nice people at DoomCenter the presentation of Doom 3 at E3 was infact running at medium quality graphics, here's a snippet:

Update: Apparently, Carmack was comparing ATI's next-gen card to NVIDIA's GeForce 4 - not the new technology that should be out in August. Also, Carmack has posted notice that the Doom 3 video at E3 was actually run at medium quality - not max. Jeez.
