She has hepatitis C. About 200 million people worldwide are infected with hep C, with about 5 million of these being in the US and another 5 million in western Europe. There is a higher incidence in eastern Europe.

Hep C sometimes lays in wait many years after exposure before rearing its head, but is usually spread through blood or blood products, from their mother or from needles previously used by an infected person. A lot of people were exposed by getting transfusions during or after surgeries performed before the mid 1990's.

PA's claim of getting it from a tatoo needle is very plausible since their re-use, and that of tatoo dye containers, was commonplace until just a few years ago and is still done by unscrupulous tatoo artists and amatures.

Getting tatoos with used needles or dye containers can also spread other diseases including HIV, hepatitis B, tetanus and tuberculosis. This is enough of a problem that the Red Cross won't let you donate blood for a year after getting a tatoo.

As per lottery or drawn positions, AFAIK, certain positions in Athens were drawn and certain were elected.

This is not at all an A. Clarke era/contemporary science fiction idea.

As per contemporary USA - it would have probably been beneficial as it would have been cheaper to install one drawing machine compared to multitude of e-voting machines, there also would have been no loss in time and funds due to campaign and it would still permeate the illusion of people's government among the lemmings.
